In consideration of my minor child/ward being allowed to participate, in any way in Amateur Sports Authority ( the "Operator") related events and activities, including but not limited to A Step Up Sports Events Management, the undersigned, the undersigned acknowledges, appreciates, and agrees that:

The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death.

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISK, both known and unknown, EVEN IF ARISING FROM the NEGLIGENCE of the "Operator", Amateur Sports Authority.

I willingly agree to comply with stated and customary terms and conditions for participation.

I for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS, Amateur Sports Authority, any personnel or vendors servicing scheduled sports events via Amateur Sports Authority.

As further consideration of participation my minor child/ward being allowed to participate, I for myself and on behalf of my heirs hereby agree to the terms of this document.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTANDS ITS TERMS, UNDERSTANDS THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
